License Fees

The annual license fee for Licensed Spiritual Healer issued by the Alliance of Christian Healers License Board is $65 for the first year and $45 per year thereafter payable as follows:

1. You may pay your initial license fee of $65.00 US by credit card using the IPX
   IPX Services payment option for initial applications.
   
   
2. You may pay your renewal license fee of $45.00 US by credit card using the IPX Services payment option for renewal applications.
   
   
3. You may pay by including your credit card information on your application form.
   
   
4. You may include your check or money order with your application.

How To Remit Your Application

Please complete either an:

1. New application for a license by Word Doc or by PDF File
   
   
2. Renewal application by Word Doc or PDF File

You may remit your application by fax, email or mail:
